- Provide compassionate and comprehensive inpatient adult palliative care to patients facing serious illness.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients.
- Perform comprehensive assessments, including physical, psychosocial, and spiritual evaluations, to identify patient needs and goals.
- Administer medications and treatments as prescribed by physicians.
- Educate patients and their families on treatment options, disease management, and end-of-life care.
- Advocate for patients and ensure their wishes and goals are respected and honored.
- Monitor and document patient progress and communicate updates to the healthcare team.
- Stay up-to-date on current research and best practices in palliative care to provide the highest quality of care to patients.
- Provide emotional support and guidance to patients and their families during difficult times.
- Collaborate with community resources to ensure continuity of care for patients after discharge.
- Adhere to all state and federal regulations and maintain confidentiality of patient information.
- Participate in quality improvement initiatives and contribute to the growth and development of the palliative care program.
- Act as a mentor and resource for other healthcare providers on the team.
- Maintain professional development through continuing education and participation in professional organizations.
- Adhere to the mission, vision, and values of MUSC Health and the Department of Medicine.
Advanced Degree In Nursing Or Physician Assistant Studies: The Ideal Candidate For This Position Should Possess A Master's Degree In Nursing Or Physician Assistant Studies From An Accredited Institution.
Current Np Or Pa License: The Candidate Must Hold A Valid And Current License As A Nurse Practitioner Or Physician Assistant In The State Of South Carolina. This Is A Mandatory Requirement For Practicing In This Role At Musc Health.
Certification In Palliative Care: The Candidate Must Hold A Certification In Palliative Care From A Recognized Organization Such As The National Board For Certification Of Hospice And Palliative Nurses (Nbchpn) Or The National Board For Certification Of Physician Assistants (Nccpa).
Minimum Of 3 Years Of Experience In Palliative Care: The Ideal Candidate Should Have At Least 3 Years Of Experience Working In Palliative Care, Preferably In An Inpatient Setting. This Experience Will Ensure That The Candidate Is Well-Equipped To Handle The Challenges Of This Role.
Strong Communication And Interpersonal Skills: The Candidate Should Possess Excellent Communication And Interpersonal Skills To Effectively Communicate And Collaborate With Patients, Families, And Healthcare Teams. This Is Essential For Providing Comprehensive And Compassionate Care To Patients In The Inpatient Adult Palliative Care Unit.
