Master's Social Worker

1. Conduct comprehensive psychosocial assessments of patients and their families to identify needs and develop care plans.
2. Provide individual and group therapy to patients and families to address mental health concerns.
3. Collaborate with interdisciplinary healthcare team to develop and implement integrated care plans for patients.
4. Advocate for patients and families to ensure their needs are met and their rights are respected.
5. Educate and support patients and families on available community resources and assist with accessing them.
6. Maintain accurate and thorough documentation of all patient interactions and progress.
7. Stay up-to-date on evidence-based practices and incorporate them into treatment plans.
8. Provide crisis intervention and support for patients and families in times of need.
9. Serve as a liaison between patients and their families and the healthcare team.
10. Participate in interdisciplinary team meetings and provide input on patient care and treatment.
11. Develop and maintain relationships with community organizations and resources to enhance patient care.
12. Adhere to all ethical and legal standards in the field of social work.
13. Continuously evaluate and monitor patient progress and adjust care plans as needed.
14. Collaborate with other social workers and provide professional support and guidance as needed.
15. Participate in ongoing training and professional development opportunities to enhance skills and knowledge.
