Hospice Chaplain - PRN

We are currently seeking a Hospice Chaplain to join our team on a PRN (as needed) basis. As a Hospice Chaplain, you will have the opportunity to provide spiritual and emotional support to patients and their loved ones during their end-of-life journey. 1. Provide spiritual and emotional support to patients and their families during their end-of-life journey.
2. Conduct spiritual assessments and develop individualized care plans for patients.
3. Offer prayer, counseling, and spiritual guidance to patients of all faiths.
4. Assist patients and families in finding peace and comfort during the dying process.
5. Collaborate with interdisciplinary team members to ensure holistic care for patients.
6. Maintain confidentiality and respect patients' beliefs and values.
7. Conduct regular visits to patients in hospice care to provide spiritual support.
8. Attend interdisciplinary team meetings and participate in care planning discussions.
9. Provide bereavement support to families and loved ones.
10. Document all interactions with patients and families accurately and timely.
11. Stay updated on current hospice care practices and attend professional development opportunities.
12. Serve as a spiritual resource for staff members and provide support to them as needed.
13. Adhere to ethical and legal standards in the provision of spiritual care.
14. Maintain a positive and compassionate attitude towards patients and their families.
