- Provide exceptional patient care by utilizing advanced pharmacotherapy skills to assess, monitor, and optimize medication therapy for patients.
- Collaborate with interdisciplinary teams to develop and implement treatment plans that address patient-specific needs and ensure optimal outcomes.
- Serve as a subject matter expert in pharmacotherapy, providing education and training to healthcare professionals, students, and patients.
- Conduct medication therapy reviews to identify and resolve drug therapy problems and improve patient safety and satisfaction.
- Participate in research activities, including data collection, analysis, and dissemination, to contribute to the advancement of medicine and patient care.
- Maintain a high level of proficiency in current pharmacy practice standards, guidelines, and regulations to ensure compliance and quality of care.
- Communicate effectively with patients, caregivers, and healthcare providers to promote medication adherence and address any concerns or questions.
- Participate in the development and implementation of departmental policies and procedures to ensure safe and efficient medication use.
- Provide leadership and mentorship to pharmacy staff and students, as well as participate in the training and orientation of new team members.
- Contribute to the development and implementation of quality improvement initiatives to enhance patient care and outcomes.
- Maintain accurate and complete documentation of patient care activities and participate in performance improvement efforts.
- Actively participate in professional development activities to stay current on the latest advancements and best practices in pharmacotherapy.
- Adhere to ethical and legal standards in all aspects of job responsibilities, including patient confidentiality, medication safety, and professional conduct.
Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited College Of Pharmacy.
Completion Of A Post-Graduate Residency Or Fellowship Program In Pharmacotherapy.
Board Certification In Pharmacotherapy (Bcps) From The Board Of Pharmacy Specialties.
Current And Unrestricted Pharmacist License In The State Of South Carolina.
Minimum Of 3 Years Of Clinical Experience In A Hospital Or Ambulatory Care Setting.
