- Collaborate with a multidisciplinary team to assess patients' medication regimens and make recommendations for changes or adjustments as needed.
- Conduct thorough medication reviews and provide patient education on proper medication usage, potential side effects, and drug interactions.
- Monitor and evaluate the effectiveness of medication therapy and make necessary interventions to ensure optimal patient outcomes.
- Serve as a resource for healthcare providers and other staff members regarding medication therapy for patients with heart failure.
- Stay current with the latest developments and research in the field of cardiology and incorporate new information into practice.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Maintain accurate and detailed patient records, including medication histories, assessments, and interventions.
- Provide clinical guidance and support to pharmacy staff, including technicians and students.
- Collaborate with other healthcare professionals, such as physicians and nurses, to ensure coordinated and comprehensive care for patients with heart failure.
- Maintain compliance with all relevant laws, regulations, and policies related to pharmacy practice.
- Participate in professional development activities, such as continuing education, to maintain and enhance clinical skills and knowledge.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy
Board Certification In Pharmacotherapy Or Cardiology From The Board Of Pharmacy Specialties
At Least 2 Years Of Clinical Pharmacy Experience In A Hospital Or Ambulatory Care Setting, With A Focus On Heart Failure Management And Treatment
Extensive Knowledge Of Evidence-Based Guidelines And Treatment Options For Heart Failure, Including Medication Management And Patient Education
Excellent Communication Skills And Ability To Collaborate With Healthcare Team Members, Including Physicians, Nurses, And Other Specialists, To Develop Optimal Care Plans For Heart Failure Patients.
