1. Provide pharmaceutical care by reviewing and assessing patient medication regimens for safety, efficacy, and appropriateness.
2. Collaborate with healthcare team members to develop and implement individualized treatment plans for patients.
3. Advise physicians, nurses, and other healthcare professionals on drug therapy and potential drug interactions.
4. Monitor patient response to medication therapy and make necessary adjustments to ensure optimal outcomes.
5. Maintain accurate and complete patient records, including medication histories and therapeutic recommendations.
