- Conduct comprehensive medication reviews and collaborate with healthcare team to optimize patient drug therapy.
- Develop and implement evidence-based protocols and guidelines for medication use in the Med/Surg department.
- Monitor patient response to medication therapy and make necessary adjustments to ensure safety and efficacy.
- Educate patients, caregivers, and healthcare team on proper medication use, potential side effects, and drug interactions.
- Work closely with physicians and other healthcare professionals to develop individualized treatment plans for patients.
- Ensure accurate and timely documentation of all patient interactions and interventions.
- Stay current with new drugs, treatment guidelines, and industry developments to provide the best possible care.
- Participate in quality improvement initiatives to enhance patient outcomes and safety.
- Collaborate with pharmacy team to ensure proper medication storage, preparation, and dispensing.
- Provide leadership and guidance to pharmacy technicians and other support staff.
- Maintain a high level of professionalism and adhere to ethical standards in all interactions.
- Participate in interdisciplinary rounds and provide medication expertise to enhance patient care.
- Maintain and promote a safe and clean work environment.
- Communicate effectively with patients, their families, and healthcare team members.
- Adhere to all hospital policies, procedures, and regulatory requirements.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Current State Pharmacy License And Registration As A Pharmacist.
Board Certification In Pharmacotherapy (Bcps) Or Other Relevant Specialty Certification.
Minimum Of 2-3 Years Of Clinical Experience In A Hospital Setting, Preferably In A Med/Surg Unit.
Strong Knowledge Of Drug Therapy, Pharmacokinetics, And Pharmacodynamics In The Acute Care Setting.
