
Clinical Pharmacist- Chronic Care Management

- Conduct comprehensive medication reviews for patients with chronic conditions to optimize their medication therapy and improve patient outcomes.
- Collaborate with other healthcare professionals, including physicians and nurses, to develop individualized care plans for patients with chronic conditions.
- Monitor patients' medication regimens and provide recommendations for adjustments as needed.
- Educate patients on their medications, potential side effects, and proper administration techniques.
- Stay up-to-date on the latest research and guidelines related to chronic care management and incorporate this knowledge into practice.
- Maintain accurate and complete patient medication records, including medication histories, allergies, and drug interactions.
- Communicate effectively with patients and their caregivers to ensure understanding of medication regimens and address any concerns or questions.
- Participate in interdisciplinary team meetings to discuss patient care and contribute expertise on medication-related issues.
- Serve as a resource for healthcare professionals and provide guidance on medication therapy for patients with chronic conditions.
- Adhere to all state and federal laws and regulations related to pharmacy practice, including maintaining proper licensure and continuing education requirements.
Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited University
Valid State Pharmacist License And Registration
Board Certification In Pharmacotherapy Or Ambulatory Care
Minimum Of 2-3 Years Of Experience In Chronic Care Management Or Ambulatory Care Pharmacy
Strong Clinical Knowledge And Expertise In Managing Chronic Diseases Such As Diabetes, Hypertension, And Cardiovascular Conditions.
