- Provide specialized care and coordination for adult heart transplant patients throughout their entire transplant journey.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients.
- Monitor and assess patients' physical and emotional well-being, and adjust care plans accordingly.
- Educate patients and their families on the transplant process, including pre- and post-operative care, medication management, and lifestyle changes.
- Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and continuity of care.
- Advocate for patients' needs and collaborate with other healthcare professionals to address any challenges or concerns.
- Maintain accurate and detailed records of patients' medical history, treatments, and progress.
- Stay current on advancements in cardiac care and transplant procedures to provide the best possible care for patients.
- Adhere to all hospital policies and procedures, as well as state and federal regulations, to ensure quality and safe patient care.
- Participate in quality improvement initiatives to continuously improve patient outcomes and experiences.
- Serve as a resource and mentor for other healthcare professionals in the field of cardiac and transplant care.
- Collaborate with community organizations and support groups to provide resources and support for patients and their families.
Active Rn License: The Ideal Candidate For An Adult Heart Transplant Rn Coordinator Position At Musc Health Should Possess An Active Registered Nurse (Rn) License In The State Of South Carolina. This Is A Requirement To Practice Nursing And Provide Direct Patient Care.
Minimum Of 2 Years Of Experience In Cardiac Nursing: Musc Health Is Looking For Candidates Who Have A Minimum Of 2 Years Of Experience In Cardiac Nursing, Preferably In A Transplant Or Critical Care Unit. This Experience Will Ensure The Candidate Has A Strong Understanding Of The Unique Needs And Challenges Of Heart Transplant Patients.
Certification In Transplant Nursing: Having A Certification In Transplant Nursing, Such As The Certified Clinical Transplant Coordinator (Cctc) Certification, Is Highly Desirable For An Adult Heart Transplant Rn Coordinator At Musc Health. This Certification Demonstrates A High Level Of Knowledge And Expertise In The Field Of Transplant Nursing.
Strong Communication And Organizational Skills: As An Adult Heart Transplant Rn Coordinator, The Candidate Will Be Responsible For Coordinating And Managing The Care Of Transplant Patients, As Well As Working Closely With A Multidisciplinary Team. Therefore, Strong Communication And Organizational Skills Are Essential For This Role.
