Job Description

Moffitt Cancer Center is seeking a motivated and compassionate Health Navigator Trainee to join our patient-centered care team. As an integral part of our mission to reduce the burden of cancer, the Health Navigator Trainee will have the opportunity to make a meaningful difference in the lives of our patients. This is a unique role in which you will be actively involved in connecting patients to resources that are essential to managing their care. The ideal candidate will be a self-starter with an unwavering commitment to patient care, demonstrate strong communication and organizational skills, and have the ability to work independently. Required Qualifications: • Bachelor’s degree in a health-related field • A minimum of one year of experience in a healthcare setting • Demonstrated excellent written and verbal communication skills• Proficiency in Microsoft Office Suite • Ability to work independently and in a team environment • Ability to provide compassionate care and understanding to patients and their families

Responsibilities:

• Provide patients with resources and connections to facilitate their access to care. • Build relationships with patients and families to promote understanding and trust. • Assess patient needs to identify and address any gaps in care. • Monitor progress, track outcomes and document patient interactions. • Assist in developing and implementing patient care plans. • Collaborate with healthcare providers and other members of the care team to ensure patient needs are met. • Utilize organizational and communication skills to provide timely and accurate information to patients and their families. • Maintain up-to-date knowledge of community resources and healthcare services. • Ensure all relevant laws and regulations are adhered to. • Provide compassionate support to patients and their families.
