At Miriam Hospital, our Social Workers have a critical role in providing comprehensive care to patients. We are looking for an experienced and compassionate individual to join our team. The ideal candidate will be a strong communicator, will have experience providing support to patients and families, and must possess a Master's degree in Social Work. As a Social Worker at Miriam Hospital, you will provide emotional and practical support to those most in need, as well as making referrals to other services and resources. You will need to be organized, reliable, and have a strong sense of empathy. Responsibilities:

Provide emotional and practical support to those in need
Assist patients and their families with navigating the healthcare system
Make referrals to other services and resources as needed
Develop and implement individualized care plans for patients
Collaborate with other healthcare professionals to ensure comprehensive care is being provided
Maintain strong communication with patients and their families
Provide crisis intervention when necessary
Utilize assessment and intervention techniques in order to provide holistic care
Remain up to date on relevant policies, procedures, and programs
Document and monitor patient progress
