
Pharmacy Ambulatory Liaison
We are looking for an experienced and passionate Pharmacy Ambulatory Liaison to join the Pharmacy team at Miriam Hospital. In this role, you will be a critical member of our team, ensuring that the highest standards of patient care are met through the delivery of quality pharmaceutical services.To be successful in this role, you must be a registered pharmacist with at least 3 years of experience in a hospital setting. You must also have excellent communication and customer service skills, as well as the ability to troubleshoot problems and think critically. You should also have strong organizational skills with the ability to prioritize multiple tasks and a commitment to excellence.If you are a motivated and knowledgeable pharmacist looking for an opportunity to make a difference in patient care, we want to hear from you!
Responsibilities:
- Develop and implement strategies to improve the quality of patient care through the delivery of pharmaceutical services
- Monitor patient medications and provide appropriate advice and guidance to staff and patients
- Evaluate and recommend changes to pharmacy practice protocols
- Collaborate with other healthcare providers to ensure the safe and effective use of medications
- Foster relationships with external customers such as managed care organizations and other healthcare providers
- Maintain accurate patient records and document medication orders
- Monitor and ensure compliance with all relevant laws, regulations and standards
- Remain current on new developments and advances in pharmacy and healthcare
- Provide education to patients and staff on medication use, storage, and disposal
- Participate in committees and task forces to drive improvement in pharmacy practice
Excellent Interpersonal And Communication Skills
Ability To Work Independently And In A Team Environment
Experience With Electronic Health Records
Bachelor's Degree In Pharmacy
Pharmd Or Equivalent
At Least Years Of Experience In A Healthcare Setting
Knowledge Of Laws And Regulations Governing Pharmacy
Knowledge Of Ambulatory Pharmacy Practice
