Job Description

Mercy is seeking a dedicated Social Worker to join our hospice team and provide support and care to patients and their families during the end-of-life journey. As a Social Worker at Mercy, you will have the opportunity to use your skills and expertise to help improve the quality of life for those facing terminal illness. We are looking for a highly skilled and motivated individual with a strong understanding of hospice care and a passion for helping others.   1. Provide emotional, social, and practical support to hospice patients and their families during the end-of-life journey.
  2. Conduct psychosocial assessments to identify patient and family needs and develop individualized care plans.
  3. Collaborate with the interdisciplinary team to ensure holistic and comprehensive care for patients.
  4. Offer counseling and support to patients and families to help them cope with the challenges of terminal illness.
  5. Facilitate communication and decision-making between patients, families, and healthcare providers.
  6. Connect patients and families with community resources and support services, such as financial assistance and bereavement support.
  7. Advocate for patients' rights and ensure their wishes and preferences are honored.
  8. Maintain accurate and timely documentation of patient care and progress.
  9. Participate in team meetings, case conferences, and educational opportunities to enhance hospice care knowledge and skills.
  10. Adhere to professional ethical standards and maintain confidentiality of patient information.

Job Qualifications
  • A Master's Degree In Social Work From An Accredited University. This Qualification Ensures That The Candidate Has Received Comprehensive Education And Training In The Field Of Social Work, Including Courses In Hospice Care.

  • At Least 2 Years Of Experience Working In End-Of-Life Care Or Hospice Settings. This Experience Demonstrates A Strong Understanding Of The Unique Needs And Challenges Faced By Patients And Families In Hospice Care.

  • State Licensure As A Licensed Clinical Social Worker (Lcsw). This Qualification Ensures That The Candidate Has Met The Necessary Requirements For Practicing Clinical Social Work, Including Completing Supervised Clinical Hours And Passing A Licensure Exam.

  • Strong Communication And Interpersonal Skills. Social Workers In Hospice Care Must Be Able To Effectively Communicate With Patients, Families, And Interdisciplinary Healthcare Teams, As Well As Provide Emotional Support And Counseling.

  • Knowledge Of Ethical And Legal Issues In Hospice Care. This Includes Understanding And Adhering To Patient Confidentiality, Informed Consent, And Advance Directives, As Well As Being Familiar With State And Federal Laws And Regulations Related To Hospice Care.
