- Provide emotional and practical support to patients and their families at the end of life.
- Conduct thorough assessments of patients' social, emotional, and practical needs.
- Collaborate with interdisciplinary team members to develop and implement comprehensive care plans.
- Advocate for patients' rights and help them navigate the healthcare system.
- Offer counseling and support services to patients and families, including grief and bereavement support.
- Assist with advanced care planning and end-of-life decision making.
- Connect patients and families with community resources and support services.
- Maintain accurate and timely documentation of patient care and progress.
- Participate in regular team meetings and case conferences to discuss patient care and progress.
- Stay updated on current hospice and social work practices and regulations.
- Collaborate with community organizations and agencies to enhance support services for patients and families.
- Provide education and support to caregivers and family members.
- Maintain a high level of empathy and compassion while working with patients and families.
- Adhere to ethical and professional standards of practice.
- Maintain confidentiality of patient information.
- Participate in quality improvement initiatives to enhance patient care.
- Handle crisis situations and provide appropriate interventions.
- Collaborate with interdisciplinary team members to provide a holistic approach to patient care.
- Adhere to all hospice policies and procedures.
- Continually work to enhance the patient and family experience during end-of-life care.
Bachelor's Or Master's Degree In Social Work From An Accredited University
State Licensure As A Licensed Clinical Social Worker (Lcsw)
Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Hospice Care
Knowledge Of Hospice Regulations, Medicare/Medicaid Guidelines, And Ethical Standards Of Practice
Strong Communication And Interpersonal Skills, With The Ability To Provide Compassionate And Empathetic Care To Patients And Their Families During End-Of-Life Care.
