  1. Provide comprehensive care management services to patients, including assessment, planning, implementation, and evaluation of their physical, emotional, and social needs.
  2. Advocate for and support patients and their families during difficult times, ensuring they have access to necessary resources and services.
  3. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
  4. Conduct regular assessments to monitor patients' progress and make necessary adjustments to their care plans.
  5. Communicate effectively and professionally with patients, families, and healthcare team members.
  6. Maintain accurate and timely documentation of patient care activities.
  7. Stay up-to-date on social work principles, policies, and regulations to provide high-quality and ethical care to patients.
  8. Identify and address any potential barriers to care and work towards solutions to improve patient outcomes.
  9. Provide crisis intervention and support to patients and their families when needed.
  10. Collaborate with community resources and organizations to connect patients with additional support services.
  11. Uphold patient confidentiality and privacy at all times.
  12. Continuously strive to improve patient care and outcomes through ongoing education and training.
  13. Adhere to all organizational policies and procedures, as well as state and federal regulations related to social work practice.
  14. Maintain a positive and professional attitude and work collaboratively with colleagues to achieve common goals.
  15. Serve as a role model for ethical and compassionate social work practice within the organization.

  • Strong Communication And Interpersonal Skills: As A Care Manager, You Will Be Working Closely With Clients, Families, And Healthcare Professionals. Excellent Communication And Interpersonal Skills Are Essential For Building Relationships, Advocating For Clients, And Collaborating With Others.
