
Registered Nurse - Hospice Community Liaison

- Serve as a representative of Mercy and the hospice program, promoting our mission and values to the community.
- Develop and maintain relationships with healthcare providers, social workers, and community organizations to educate them about our hospice services and refer potential patients to our program.
- Meet with patients and their families to provide information and support, assess their needs, and determine if they are eligible for hospice care.
- Collaborate with the hospice team to create individualized care plans for patients and coordinate their care with other healthcare providers.
- Communicate regularly with patients, families, and healthcare providers to ensure continuity of care and address any concerns or questions.
- Participate in community outreach and education events to raise awareness about end-of-life care and our hospice services.
- Maintain accurate and up-to-date records of patient interactions, referrals, and community outreach activities.
- Stay informed about industry trends and best practices in hospice care and use this knowledge to improve our services and outreach strategies.
- Participate in team meetings and contribute to the development of strategic plans and goals for the hospice program.
- Adhere to all legal and ethical standards, including HIPAA regulations, in all interactions and communications.
- Act as a liaison between the hospice program, patients, families, and healthcare providers to ensure effective communication and coordination of care.
- Represent Mercy and the hospice program at professional conferences and networking events to promote our services and build relationships.
- Provide support and guidance to patients and families during difficult end-of-life decisions and help them navigate the hospice care process.
- Continuously evaluate and monitor the effectiveness of our outreach and referral strategies and make recommendations for improvements.
- Adhere to all policies and procedures of Mercy and the hospice program and maintain confidentiality of all patient information.
Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Typically Required For A Registered Nurse - Hospice Community Liaison Position At Mercy. This Degree Provides A Strong Foundation In Nursing Principles And Prepares Individuals For A Wide Range Of Nursing Roles.
Active Registered Nurse (Rn) License: Candidates Must Have An Active Rn License In The State Where They Will Be Working. This License Is Obtained By Passing The National Council Licensure Examination (Nclex) After Completing An Accredited Nursing Program.
Experience In Hospice Care: Mercy May Prefer Candidates Who Have Previous Experience Working In A Hospice Setting. This Experience Allows The Candidate To Have A Better Understanding Of Hospice Care And The Unique Needs Of Patients And Their Families.
Strong Communication And Interpersonal Skills: As A Hospice Community Liaison, The Individual Will Be Responsible For Building Relationships With Community Organizations, Healthcare Providers, And Patients And Their Families. Therefore, Strong Communication And Interpersonal Skills Are Essential For This Role.
Knowledge Of Medicare And Medicaid Regulations: A Thorough Understanding Of Medicare And Medicaid Regulations Is Important For This Role, As Hospice Care Is Often Funded Through These Programs. A Candidate With Prior Experience In Navigating These Regulations Will Be Better Equipped To Serve As A Liaison Between Mercy And The Community.
