Job Description

In this role, you will serve as a liaison between our hospice program and the community, fostering relationships and promoting our services to provide the best care possible.   1. Serve as a liaison between the hospice program and the community to promote and educate about the services offered.
  2. Build and maintain relationships with community organizations, healthcare providers, and other referral sources.
  3. Educate healthcare professionals and the community about hospice care and its benefits.
  4. Collaborate with the hospice team to develop and implement community outreach programs and events.
  5. Identify and pursue opportunities for collaboration and partnerships with community organizations to enhance the hospice program.
  6. Conduct presentations and informational sessions to raise awareness and understanding of hospice care.
  7. Participate in networking events and conferences to represent the hospice program and build relationships with potential partners.
  8. Conduct regular visits and follow-ups with healthcare providers and community organizations to ensure a positive and ongoing relationship.
  9. Keep up-to-date with industry trends and changes in hospice care to effectively educate and advocate for the hospice program.
  10. Maintain accurate records of community outreach activities and report on progress and outcomes to the hospice team.
  11. Support the hospice team in identifying and addressing any barriers to hospice care in the community.
  12. Collaborate with the marketing team to develop and distribute informational materials about the hospice program.
  13. Maintain a positive and professional image of the hospice program at all times.
  14. Adhere to all privacy and confidentiality regulations when interacting with patients, families, and community partners.
  15. Serve as a resource and point of contact for community members seeking information about hospice care.

Job Qualifications
  • Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Required For This Position, As It Demonstrates A Strong Foundation In Nursing Knowledge And Skills.

  • Current Rn License: The Candidate Must Be A Registered Nurse With A Current And Active License In The State Where The Job Is Located.

  • Experience In Hospice Care: A Minimum Of 2-3 Years Of Experience In Hospice Care Is Preferred For This Role, As It Requires A Deep Understanding Of The Unique Needs And Challenges Of Hospice Patients.

  • Excellent Communication And Interpersonal Skills: The Role Of A Hospice Community Liaison Involves Working With A Variety Of Stakeholders, Including Patients, Families, Healthcare Providers, And Community Organizations. Therefore, Strong Communication And Interpersonal Skills Are Essential For Success In This Role.

  • Knowledge Of Community Resources: The Ideal Candidate Should Have A Good Understanding Of The Local Healthcare Landscape And Be Familiar With Community Resources For Hospice Patients And Their Families. This Knowledge Will Be Critical In Connecting Patients With The Appropriate Services And Support.
