- Coordinate and manage care for patients at Mercy, ensuring their needs are met in a compassionate and high-quality manner.
- Serve as the primary point of contact for patients and their families, providing support and guidance throughout their care journey.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans for patients.
- Monitor and assess patient progress and make necessary adjustments to the care plan as needed.
- Educate patients and their families on their conditions, treatments, and self-care techniques.
- Facilitate communication between patients, families, and healthcare team members to ensure continuity of care.
- Stay up-to-date on industry best practices and new developments in healthcare to continuously improve patient care.
- Maintain accurate and detailed patient records and ensure compliance with all regulatory standards.
- Advocate for the needs and rights of patients, ensuring they receive the best possible care.
- Collaborate with insurance companies and other third-party payers to ensure proper coverage for patient care.
- Provide support and guidance to other healthcare team members, including mentoring and training new staff.
- Participate in quality improvement initiatives and provide suggestions for enhancing patient care processes.
- Adhere to all hospital policies and procedures, including safety protocols and HIPAA regulations.
- Maintain a positive and professional attitude, embodying the values of Mercy in all interactions with patients, families, and colleagues.
- Continuously strive to improve and enhance patient satisfaction and overall experience at Mercy.
Bachelor's Degree In Nursing From An Accredited Institution
Current And Valid Registered Nurse (Rn) License In The State Of Employment
Minimum Of 2-3 Years Of Clinical Experience In A Healthcare Setting, Preferably In Case Management Or Care Coordination
Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Multidisciplinary Teams
Knowledge And Proficiency In Electronic Medical Records And Healthcare Technology To Effectively Manage Patient Care Plans And Documentation.
