Job Description

We are currently seeking a dedicated and empathetic Patient Support Associate to join our Inpatient Care Management team. In this role, you will play a vital role in ensuring our patients receive the best possible care and support during their stay with us.   1. Greet and welcome patients and their families upon admission to the facility.
  2. Provide emotional support and a compassionate presence for patients during their stay.
  3. Assist patients with daily tasks and activities, such as meal ordering, transportation, and personal hygiene.
  4. Communicate effectively with patients, families, and healthcare team members to ensure a smooth and positive patient experience.
  5. Collaborate with nurses and other healthcare professionals to address patient needs and concerns.
  6. Monitor patient comfort and safety, and report any changes or issues to the appropriate staff.
  7. Maintain accurate and confidential patient records and documentation.
  8. Serve as a liaison between patients, families, and healthcare providers to facilitate communication and address any concerns or questions.
  9. Provide education and resources to patients and families regarding the facility, services, and resources available to them.
  10. Adhere to all facility policies and procedures, as well as state and federal regulations, to ensure patient safety and confidentiality.
  11. Maintain a clean and organized patient environment to promote a comfortable and healing atmosphere.
  12. Continuously strive to improve patient satisfaction and ensure a positive patient experience.
  13. Attend and actively participate in team meetings and trainings to stay informed of changes and updates in patient care practices.
  14. Act as an advocate for patients and their families, ensuring their needs and rights are respected and met.

Job Qualifications
  • A Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Patient Support Or Care Management Roles.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Communicate With Patients, Families, And Healthcare Professionals.

  • Knowledge Of Medical Terminology And Experience With Electronic Health Records.

  • Ability To Multitask And Prioritize Tasks In A Fast-Paced Environment.

  • Demonstrated Empathy And Compassion Towards Patients And Their Families, With A Commitment To Providing High-Quality Care And Support.
