As a Case Manager-Therapist, you will play a vital role in providing integrated mental health services to our patients, working closely with a multidisciplinary team to ensure the best possible care.   1. Provide compassionate and comprehensive care to patients struggling with mental health issues.
  2. Work as part of a multidisciplinary team to provide integrated mental health services.
  3. Conduct thorough assessments and develop individualized treatment plans for patients.
  4. Utilize evidence-based practices and treatment modalities to promote healing and wellness.
  5. Monitor and track patient progress, making adjustments to treatment plans as needed.
  6. Collaborate with other healthcare professionals to ensure coordinated care for patients.
  7. Maintain accurate and up-to-date patient records and documentation.
  8. Communicate effectively with patients, families, and other team members.
  9. Advocate for patients and assist with accessing community resources and support.
  10. Stay up-to-date on current research and best practices in the field of mental health.
  11. Adhere to ethical and professional standards in all aspects of job performance.
  12. Participate in ongoing training and professional development opportunities.
  13. Maintain confidentiality and adhere to HIPAA regulations.
  14. Assist with crisis intervention and management as needed.
  15. Foster a positive and supportive work environment for colleagues and team members.
  16. Demonstrate a commitment to the mission and values of Mercy.
  17. Act as a role model for patients and colleagues, displaying empathy, compassion, and professionalism.
  18. Continuously evaluate and improve upon case management and therapy practices.
  19. Attend and contribute to team meetings and case conferences.
  20. Other duties as assigned by supervisor.

Job Qualifications
  • Master's Degree In Social Work, Counseling, Or Related Field

  • Valid State Licensure As A Mental Health Therapist (Lcsw, Lpc, Lmft)

  • Minimum Of 3 Years Of Experience In Case Management And Therapy

  • Knowledge And Understanding Of Evidence-Based Practices For Mental Health Treatment

  • Excellent Communication And Interpersonal Skills, Including Ability To Collaborate With Interdisciplinary Team Members And Provide Culturally Sensitive Care.
