  • Conduct comprehensive assessments of clients' mental health needs and develop individualized treatment plans in collaboration with clients and their support systems
  • Monitor and evaluate clients' progress towards treatment goals, making necessary adjustments to ensure optimal outcomes
  • Provide ongoing support and assistance to clients, including coordinating appointments, connecting them with community resources, and advocating for their needs
  • Collaborate with interdisciplinary team members, including therapists, doctors, and social workers, to ensure coordinated and holistic care for clients
  • Maintain accurate and timely documentation of all client interactions and progress in electronic health records
  • Participate in regular team meetings and case conferences to discuss clients' progress and identify any challenges or barriers
  • Stay up-to-date on best practices and evidence-based interventions for mental health care to continuously improve the quality of services provided
  • Adhere to all ethical and legal standards in the provision of mental health care services
  • Provide crisis intervention as needed, including assessing risk and making appropriate referrals for clients in urgent situations
  • Maintain confidentiality and respect clients' rights to privacy at all times
  • Participate in professional development activities and seek opportunities for growth and advancement within the organization.

Job Qualifications
  • Excellent Communication And Interpersonal Skills

  • Bachelor's Degree In Social Work, Psychology, Or Related Field

  • Minimum Of 1-2 Years Of Experience In Mental Health Case Management
