
Mental Health Case Manager I - PRN

- Conduct comprehensive assessments of clients' mental health needs and develop individualized treatment plans in collaboration with clients and their support systems
- Monitor and evaluate clients' progress towards treatment goals, making necessary adjustments to ensure optimal outcomes
- Provide ongoing support and assistance to clients, including coordinating appointments, connecting them with community resources, and advocating for their needs
- Collaborate with interdisciplinary team members, including therapists, doctors, and social workers, to ensure coordinated and holistic care for clients
- Maintain accurate and timely documentation of all client interactions and progress in electronic health records
- Participate in regular team meetings and case conferences to discuss clients' progress and identify any challenges or barriers
- Stay up-to-date on best practices and evidence-based interventions for mental health care to continuously improve the quality of services provided
- Adhere to all ethical and legal standards in the provision of mental health care services
- Provide crisis intervention as needed, including assessing risk and making appropriate referrals for clients in urgent situations
- Maintain confidentiality and respect clients' rights to privacy at all times
- Participate in professional development activities and seek opportunities for growth and advancement within the organization.
Excellent Communication And Interpersonal Skills
Bachelor's Degree In Social Work, Psychology, Or Related Field
Minimum Of 1-2 Years Of Experience In Mental Health Case Management
Strong Understanding Of Mental Health Diagnoses And Treatment Options
Ability To Work Independently And Collaboratively In A Fast-Paced Environment
