  1. Conduct initial screenings with patients seeking services at Mercy.
  2. Gather and accurately record patient information and medical history.
  3. Assist in determining patient eligibility for services and programs.

  5. Maintain strict confidentiality of patient information and adhere to HIPAA guidelines.
  6. Communicate effectively with patients, healthcare professionals, and other team members.
  7. Ensure a smooth and efficient intake process for all patients.

  9. Provide resources and referrals to patients as needed.
  10. Stay up-to-date on all relevant healthcare and social service policies and procedures.

  12. Participate in training and professional development opportunities to enhance skills and knowledge.

  14. Handle difficult or sensitive situations with professionalism and sensitivity.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 2 Years Experience In A Similar Role, Preferably In A Healthcare Setting.

  • Strong Knowledge Of Community Resources And Social Service Programs.

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport With Diverse Populations.

  • Demonstrated Ability To Handle Confidential And Sensitive Information With Professionalism And Discretion.
