Job Description

Welcome to the Mercy South Breast Center, where we strive to provide exceptional care for every patient. We are currently seeking a dedicated and compassionate High Risk Nurse Navigator to join our team. In this role, you will play a crucial role in guiding patients through their breast cancer journey, ensuring they receive the best care possible.   1. Serve as a primary point of contact for high-risk breast cancer patients, providing support and guidance throughout their treatment and recovery process.
  2. Conduct thorough assessments of patients' medical history, risk factors, and current needs to develop individualized care plans.
  3. Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive and timely care for patients.
  4. Educate patients and their families on breast cancer prevention, early detection, and treatment options.
  5. Provide emotional support and counseling to patients and families, addressing their concerns and helping them cope with the challenges of a breast cancer diagnosis.
  6. Facilitate communication and coordination between patients and healthcare providers, ensuring all necessary appointments, tests, and treatments are scheduled and completed.
  7. Monitor patients' progress and make adjustments to care plans as needed, advocating for their needs and preferences.
  8. Maintain accurate documentation of patient interactions, treatment plans, and outcomes in accordance with hospital policies and procedures.
  9. Stay updated on the latest advancements and best practices in breast cancer care, incorporating them into patient education and treatment plans.
  10. Uphold the values and mission of the Mercy South Breast Center, providing compassionate and high-quality care to all patients.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution: A High Risk Nurse Navigator At Mercy South Must Possess A Minimum Of A Bachelor's Degree In Nursing. This Ensures That The Nurse Has A Strong Foundation In Nursing Principles And Is Equipped With The Skills And Knowledge Necessary To Provide High-Quality Care.

  • Registered Nurse (Rn) Licensure: The High Risk Nurse Navigator Must Hold A Current And Valid Rn License In The State Of Employment. This Guarantees That The Nurse Has Met The State's Requirements For Practicing Nursing And Is Legally Authorized To Provide Care To Patients.

  • Certification In Oncology Nursing: Mercy South Seeks A High Risk Nurse Navigator Who Is Certified In Oncology Nursing. This Certification Demonstrates The Nurse's Expertise In Caring For Patients With Cancer, Including Breast Cancer, And Their Ability To Provide Specialized Care To High-Risk Patients.

  • Minimum Of 3 Years Of Nursing Experience: The Ideal Candidate For The High Risk Nurse Navigator Position At Mercy South Should Have A Minimum Of 3 Years Of Nursing Experience, Preferably In An Oncology Or Breast Center Setting. This Experience Ensures That The Nurse Has A Solid Understanding Of The Complexities Of Caring For High-Risk Patients And Is Capable Of Handling The Challenges Of The Role.

  • Strong Communication And Patient Education Skills: As A High Risk Nurse Navigator, Strong Communication And Patient Education Skills Are Essential. The Nurse Must Be Able To Effectively Communicate With Patients And Their Families, As Well As Other Members Of The Healthcare Team, To Provide Comprehensive And Coordinated Care. This Includes Educating Patients On Their Condition, Treatment Options, And Self-Care Strategies.
