As a Community Health Worker, you will play a crucial role in promoting and improving the health and well-being of individuals and families within our community.   1. Conduct outreach and engage with community members to assess their health needs and provide resources and support.
  2. Collaborate with healthcare professionals to develop and implement health education programs and initiatives.
  3. Serve as a liaison between community members and healthcare providers to ensure equitable access to healthcare services.
  4. Provide individualized support and guidance to community members, including navigating healthcare systems and addressing social determinants of health.
  5. Conduct home visits and health assessments to identify and address health concerns.
  6. Maintain accurate and detailed records of community interactions and services provided.
  7. Participate in community events and meetings to promote health awareness and education.
  8. Continuously evaluate and update program strategies to meet the changing needs of the community.
  9. Serve as an advocate for underserved populations and work towards reducing health disparities.
  10. Adhere to all ethical and professional standards in the delivery of services.
  11. Collaborate with other community organizations and agencies to coordinate and enhance services.
  12. Maintain confidentiality of all client information and adhere to HIPAA regulations.
  13. Attend trainings and workshops to stay current on community health trends and best practices.
  14. Demonstrate compassion, empathy, and cultural sensitivity in all interactions with community members.
  15. Represent the organization in a positive and professional manner at all times.

Job Qualifications
  • Minimum Of A Bachelor's Degree In Public Health, Community Health, Or Related Field.

  • At Least 2 Years Of Experience Working In A Community Health Setting, Preferably With Diverse Populations.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Engage And Educate Individuals And Groups.

  • Knowledge Of Health Disparities And Social Determinants Of Health, As Well As Cultural Competency And Sensitivity.

  • Ability To Work Independently And As Part Of A Multidisciplinary Team, With Strong Organizational And Time Management Skills.
