- Build and maintain relationships with community members, particularly those who are underserved, to understand their health needs and concerns.
- Educate community members on healthcare resources and services available to them, including preventative care and health education programs.
- Collaborate with healthcare providers and community organizations to develop and implement strategies for improving access to healthcare for underserved populations.
- Conduct outreach and engage with community members to promote health and wellness initiatives and programs.
- Assist community members in navigating the healthcare system, including scheduling appointments, obtaining insurance coverage, and accessing transportation to appointments.
- Advocate for the needs and rights of community members, particularly those who are marginalized or face barriers to healthcare.
- Provide culturally sensitive and appropriate support and resources to community members, recognizing and respecting their diverse backgrounds and experiences.
- Document and track community members' progress and outcomes to measure the impact of community health programs.
- Collaborate with other healthcare professionals, including nurses, social workers, and physicians, to ensure coordinated care for community members.
- Stay up-to-date on current healthcare policies, procedures, and resources to effectively support community members.
- Maintain confidentiality and adhere to ethical standards when working with community members.
- Participate in training and professional development opportunities to enhance knowledge and skills in community health.
- Demonstrate a commitment to Mercy's mission and values of compassion, excellence, and stewardship in all interactions and activities.
Bachelor's Degree In Public Health Or Related Field: A Strong Educational Foundation In Public Health And Community Health Principles Is Essential For A Community Health Worker Position At Mercy.
Experience Working In A Community Health Setting: Previous Experience Working In A Community Health Setting, Such As A Community Health Clinic Or Non-Profit Organization, Will Demonstrate The Candidate's Understanding Of The Unique Needs And Challenges Of Underserved Populations.
Bilingual Proficiency: Mercy Serves A Diverse Patient Population, So Proficiency In A Second Language, Such As Spanish, Is Highly Desirable For A Community Health Worker Role.
Knowledge Of Local Resources And Services: Community Health Workers At Mercy Must Have A Thorough Understanding Of The Local Healthcare Landscape, Including Available Resources And Services For Low-Income And Marginalized Communities.
Strong Communication And Interpersonal Skills: As A Liaison Between Patients And Healthcare Providers, Strong Communication And Interpersonal Skills Are Essential For A Community Health Worker At Mercy.
