- Conduct outreach and build relationships within the community to identify individuals and families in need of health and wellness support.
- Provide education and resources to community members on health promotion, disease prevention, and management of chronic conditions.
- Collaborate with healthcare providers and community organizations to coordinate and facilitate access to healthcare services.
- Advocate for the needs of the community and address social determinants of health.
- Develop and implement culturally appropriate health education programs and initiatives.
- Conduct assessments to identify health needs and develop individualized care plans.
- Maintain accurate and confidential records of interactions and services provided.
- Participate in training and professional development opportunities to enhance knowledge and skills.
- Collaborate with colleagues and healthcare partners to evaluate the effectiveness of programs and services.
- Communicate regularly with supervisor and team members to share updates, challenges, and successes.
- Serve as a liaison between community members and healthcare providers to ensure continuity of care.

Strong Communication And Interpersonal Skills
