  1. Conduct outreach and build relationships within the community to identify individuals and families in need of health and wellness support.
  2. Provide education and resources to community members on health promotion, disease prevention, and management of chronic conditions.
  3. Collaborate with healthcare providers and community organizations to coordinate and facilitate access to healthcare services.
  4. Advocate for the needs of the community and address social determinants of health.
  5. Develop and implement culturally appropriate health education programs and initiatives.
  6. Conduct assessments to identify health needs and develop individualized care plans.
  7. Maintain accurate and confidential records of interactions and services provided.
  8. Participate in training and professional development opportunities to enhance knowledge and skills.
  9. Collaborate with colleagues and healthcare partners to evaluate the effectiveness of programs and services.
  10. Communicate regularly with supervisor and team members to share updates, challenges, and successes.
  11. Serve as a liaison between community members and healthcare providers to ensure continuity of care.
  12. Demonstrate a commitment to the mission and values of the organization.
  13. Adhere to all ethical and professional standards in the delivery of services.
  14. Continuously seek ways to improve and enhance the services provided to the community.
  15. Maintain a positive and professional attitude while promoting a welcoming and inclusive environment for all community members.
