Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program
Board Certification In Pharmacotherapy (Bcps) Or Critical Care (Bcccp)
Completion Of A Clinical Pharmacy Residency Or Fellowship Program, Preferably In Critical Care
Current And Unrestricted State Pharmacy License
Minimum Of 2 Years Of Experience As A Clinical Pharmacy Specialist In A Critical Care Setting, Preferably In A Hospital Environment
