  1. Coordinate and manage the care of patients at Mercy, ensuring that they receive the best possible care and support.
  2. Serve as the primary point of contact for patients, their families, and healthcare providers to ensure effective communication and coordination of care.
  3. Develop and implement care plans for patients based on their individual needs and goals.
  4. Monitor and assess the progress and effectiveness of care plans, making necessary adjustments as needed.
  5. Collaborate with interdisciplinary teams to ensure a holistic approach to patient care.
  6. Advocate for patients and their families, ensuring that their needs and preferences are taken into consideration in their care.
  7. Utilize evidence-based practices and resources to provide the highest quality of care.
  8. Maintain accurate and timely documentation of patient care and progress.
  9. Educate patients and their families on their care plans, medications, and other relevant information.
  10. Assist with discharge planning and ensure a smooth transition of care for patients.
  11. Stay up-to-date with industry standards and regulations related to patient care.
  12. Demonstrate compassion, empathy, and professionalism in all interactions with patients, families, and colleagues.
  13. Participate in team meetings and contribute to the development of new initiatives and improvements to patient care.
  14. Adhere to confidentiality and ethical standards in accordance with HIPAA regulations.
  15. Attend training and development programs to enhance skills and knowledge related to the role.

Job Qualifications
  • Knowledge Of Healthcare Regulations, Policies, And Procedures.

  • Bachelor's Or Master's Degree In Social Work, Nursing, Or A Related Field.

  • Minimum Of 3-5 Years Of Experience In Healthcare Or Social Services, Preferably In A Management Or Supervisory Role.

  • Strong Leadership And Communication Skills, With The Ability To Effectively Manage A Team And Collaborate With Other Departments.

  • Compassionate And Empathetic Attitude Towards Patients And Their Families, With A Commitment To Providing High-Quality Care And Support.
