- Coordinate and manage the care of patients at Mercy, ensuring that they receive the best possible care and support.
- Serve as the primary point of contact for patients, their families, and healthcare providers to ensure effective communication and coordination of care.
- Develop and implement care plans for patients based on their individual needs and goals.
- Monitor and assess the progress and effectiveness of care plans, making necessary adjustments as needed.
- Collaborate with interdisciplinary teams to ensure a holistic approach to patient care.
- Advocate for patients and their families, ensuring that their needs and preferences are taken into consideration in their care.
- Utilize evidence-based practices and resources to provide the highest quality of care.
- Maintain accurate and timely documentation of patient care and progress.
- Educate patients and their families on their care plans, medications, and other relevant information.
- Assist with discharge planning and ensure a smooth transition of care for patients.
- Stay up-to-date with industry standards and regulations related to patient care.
- Demonstrate compassion, empathy, and professionalism in all interactions with patients, families, and colleagues.
- Participate in team meetings and contribute to the development of new initiatives and improvements to patient care.
- Adhere to confidentiality and ethical standards in accordance with HIPAA regulations.
- Attend training and development programs to enhance skills and knowledge related to the role.
Knowledge Of Healthcare Regulations, Policies, And Procedures.
Bachelor's Or Master's Degree In Social Work, Nursing, Or A Related Field.
Minimum Of 3-5 Years Of Experience In Healthcare Or Social Services, Preferably In A Management Or Supervisory Role.
Strong Leadership And Communication Skills, With The Ability To Effectively Manage A Team And Collaborate With Other Departments.
