  1. Serve as the main point of contact for employees seeking resources and support for their health and wellbeing.
  2. Develop and maintain relationships with community organizations, healthcare providers, and other resources to connect employees with the necessary support.
  3. Conduct needs assessments and develop personalized plans for employees to improve their health.
  4. Educate and inform employees about available resources and programs to promote wellness and prevent illness.
  5. Collaborate with HR and other departments to implement wellness initiatives and programs for employees.
  6. Keep accurate records of employee interactions and track progress towards achieving health goals.
  7. Provide ongoing support and follow-up to employees to ensure they are utilizing resources and making progress towards their health goals.
  8. Participate in community outreach events and engage with community members to promote the company's commitment to health and wellbeing.
  9. Stay up-to-date on current healthcare trends and resources to continually improve the support provided to employees.
  10. Act as a liaison between employees and healthcare providers to ensure seamless communication and coordination of care.
  11. Collaborate with management to identify areas for improvement and make recommendations for enhancing the company's wellness culture.
  12. Maintain confidentiality and adhere to all HIPAA regulations when handling employee information.
  13. Attend trainings and conferences to continually develop knowledge and skills related to community health and wellness.
  14. Uphold the company's values and mission to promote the health and wellbeing of our community.

Job Qualifications
  • Bachelor's Degree In Public Health, Social Work, Or A Related Field

  • Minimum Of 2 Years Experience In Community Health Work Or Employee Engagement/Advocacy

  • Knowledge Of Healthcare Systems And Resources Available To Underserved Populations

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Relationships And Collaborate With Diverse Groups Of People

  • Bilingual Proficiency In English And Spanish Preferred, To Effectively Communicate With A Diverse Workforce.
