As a leader in this innovative program, you will oversee a team of nurses providing high-quality, in-home care to patients who would benefit from hospital-level care in the comfort of their own homes.   1. Manage and oversee a team of nurses providing in-home care to patients enrolled in the Home Hospital program.
  2. Ensure the delivery of high-quality, patient-centered care in accordance with hospital policies and procedures.
  3. Develop and implement strategies to improve patient outcomes and satisfaction.
  4. Monitor and track patient progress and make necessary adjustments to care plans.
  5. Collaborate with interdisciplinary team members to coordinate and optimize patient care.
  6. Conduct regular performance evaluations and provide ongoing support and guidance to nursing staff.
  7. Maintain accurate and up-to-date documentation of patient care and ensure compliance with regulatory requirements.
  8. Manage and allocate resources effectively to meet the needs of the Home Hospital program.
  9. Serve as a liaison between patients, families, and healthcare providers to address any concerns or issues.
  10. Participate in the development and implementation of new initiatives and programs to improve patient care and outcomes.
  11. Stay informed about current trends and best practices in home healthcare and implement them into the Home Hospital program.
  12. Foster a positive and collaborative work environment to promote teamwork and professional growth among team members.
  13. Uphold the hospital's mission and values in all aspects of the role.
  14. Attend and actively participate in meetings and trainings as required.
  15. Adhere to all hospital policies and procedures, including those related to safety and infection control.
  16. Continuously assess and improve processes and procedures to enhance the efficiency and effectiveness of the Home Hospital program.
  17. Serve as a role model for nursing staff by demonstrating compassion, empathy, and professionalism in all interactions with patients and their families.

Job Qualifications
  • Bachelor's Degree In Nursing (Bsn) From An Accredited Institution

  • Current And Active Registered Nurse (Rn) License In The State Of Massachusetts

  • Minimum Of 5 Years Of Clinical Nursing Experience, With At Least 2 Years In A Leadership Or Management Role

  • Knowledge And Understanding Of Home Hospital Care Models And Regulatory Requirements

  • Excellent Communication, Organizational, And Interpersonal Skills, With The Ability To Lead And Manage A Team Effectively.
