  1. Oversee and manage the care of patients in the Home Hospital program, ensuring high-quality and personalized care is being delivered.
  2. Promote a seamless transition from hospital to home for patients, collaborating with interdisciplinary teams to ensure continuity of care.
  3. Utilize strong clinical knowledge and skills to provide guidance and support to nursing staff, ensuring best practices and standards of care are met.
  4. Lead and mentor a team of nurses, providing ongoing training and development opportunities to promote a culture of excellence and innovation.
  5. Collaborate with healthcare providers and community resources to develop and implement individualized care plans for patients.
  6. Monitor and evaluate patient outcomes and identify areas for improvement, implementing strategies to enhance patient satisfaction and outcomes.
  7. Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and understanding of care plans.
  8. Maintain accurate and up-to-date patient records and documentation, adhering to hospital policies and procedures.
  9. Participate in performance evaluations and provide feedback to nursing staff, recognizing and addressing any areas for improvement.
  10. Stay current with the latest advancements and best practices in home healthcare, incorporating them into the Home Hospital program to continuously improve patient care.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing From An Accredited Institution

  • Current And Active Registered Nurse License In The State Of Massachusetts

  • Minimum Of 5 Years Of Clinical Experience In A Hospital Setting, Preferably In A Home Hospital Or Community Health Setting

  • Previous Experience In A Leadership Or Management Role, With Strong Communication And Team-Building Skills

  • Knowledge And Understanding Of Healthcare Regulations And Policies, As Well As Proficiency In Electronic Medical Record Systems.
