- Serve as a primary point of contact for patients and their families, providing guidance and support throughout their medication management journey.
- Build strong relationships with patients, gaining their trust and confidence in navigating their treatment plans.
- Educate patients on medication regimens, potential side effects, and proper usage to ensure optimal treatment outcomes.
- Collaborate with healthcare providers, including pharmacists and physicians, to develop and implement individualized treatment plans for patients.
- Conduct thorough assessments of patients' medication needs and provide recommendations for alternative options when necessary.
- Help patients navigate insurance coverage and financial assistance programs to ensure access to necessary medications.
- Monitor and track patients' progress and provide ongoing support and guidance to address any barriers to adherence.
- Maintain accurate and confidential patient records and documentation of interactions and interventions.
- Stay up-to-date on the latest developments and advancements in medication management and participate in ongoing training and education opportunities.
- Demonstrate a commitment to providing exceptional care and service to patients and their families in accordance with hospital policies and procedures.
Bachelor's Degree In Pharmacy Or Related Field, With A Strong Understanding Of Medication Management, Patient Education, And Healthcare Systems.
Minimum Of 2 Years Experience In A Clinical Or Community Pharmacy Setting, With A Focus On Ambulatory Care Services.
Knowledge Of Healthcare Policies And Regulations, Including Medicare And Medicaid, As Well As Insurance Billing And Reimbursement Processes.
Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport With Patients And Healthcare Professionals.
Proficient In Electronic Health Record Systems And Other Relevant Software, With The Ability To Input And Retrieve Patient Information Accurately And Efficiently.
