- Serve as a liaison between patients, pharmacists, and other healthcare professionals.
- Conduct patient consultations to ensure understanding of medications and potential side effects.

- Collaborate with healthcare team to develop and implement patient care plans.
- Educate patients on the proper use of medications and potential drug interactions.

- Adhere to all safety and regulatory standards and protocols.
- Participate in ongoing training and development to enhance skills and knowledge.
- Collaborate with other patient navigators and healthcare professionals to share best practices and improve patient outcomes.
- Act as a resource for patients and their families, addressing any questions or concerns they may have about their medications.
- Maintain a high level of organization and attention to detail in all aspects of the role.
- Demonstrate a strong commitment to patient advocacy and the mission of Massachusetts General Hospital.
Bachelor's Degree In Nursing, Pharmacy, Or Healthcare-Related Field.
Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In A Pharmacy Or Ambulatory Care Setting.
Knowledge Of Medication Therapy And Ability To Provide Medication Counseling And Education To Patients.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Healthcare Team Members And Patients From Diverse Backgrounds.
