- Conduct annual wellness visits for patients, including comprehensive health assessments and screenings.

- Educate patients on preventive care and healthy lifestyle choices to promote overall wellness.
- Utilize clinical expertise to identify and address any potential health concerns or issues.
- Maintain accurate and timely documentation of patient visits and care plans.
- Communicate effectively with patients, families, and other healthcare team members to provide exceptional care.
- Participate in interdisciplinary team meetings and contribute to the development and implementation of quality improvement initiatives.
- Stay current with healthcare trends and developments in primary care and wellness to continuously improve patient care.
- Adhere to all hospital policies, procedures, and safety regulations.
- Demonstrate compassion, empathy, and professionalism in all interactions with patients and colleagues.
Current Licensure And Certification As A Nurse Practitioner Or Physician Assistant In The State Of Massachusetts.
Advanced Degree In Nursing Or Physician Assistant Studies, With A Focus On Primary Care And Preventative Medicine.
Minimum Of 2-3 Years Of Experience Working In A Primary Care Setting, Preferably In An Outpatient Clinic Or Community Health Center.
Strong Clinical Assessment And Critical Thinking Skills, With The Ability To Diagnose And Treat A Wide Range Of Acute And Chronic Conditions.
Excellent Communication And Patient Education Skills, With A Focus On Promoting Wellness And Preventive Care For Patients Of All Ages.
