  1. Build and maintain relationships with community members, organizations, and healthcare providers to promote health and wellness initiatives.
  2. Conduct outreach and engage with individuals and families in the community to identify health needs and barriers to accessing care.
  3. Provide education and resources to community members on preventative health measures, disease management, and healthy lifestyle choices.
  4. Assist individuals and families in navigating the healthcare system and connecting them with appropriate services and resources.
  5. Collaborate with healthcare providers and community organizations to develop and implement programs and interventions to address community health needs.
  6. Conduct home visits and accompany community members to appointments as needed.
  7. Document and maintain accurate records of all interactions and services provided.
  8. Participate in training and professional development opportunities to enhance knowledge and skills related to community health.
  9. Act as an advocate for community members, addressing social and systemic barriers to healthcare access.
  10. Maintain confidentiality and adhere to all ethical and professional standards.
  11. Continuously evaluate and assess the effectiveness of community health programs and make recommendations for improvement.
  12. Attend community events and meetings to represent the hospital and promote health and wellness initiatives.
  13. Collect and analyze data to track progress and outcomes of community health programs.
  14. Stay up-to-date on current trends and best practices in community health and share knowledge with colleagues.
  15. Serve as a positive role model and ambassador for the hospital and its mission to improve community health.

Job Qualifications
  • Bachelor's Degree In Public Health, Community Health, Or A Related Field.

  • At Least 2 Years Of Experience Working In A Community Health Setting.

  • Strong Communication And Interpersonal Skills, Including The Ability To Work With Diverse Populations.

  • Knowledge Of Local Health Resources And Community Organizations.

  • Certification As A Community Health Worker Or Willingness To Obtain Certification Within 6 Months Of Hire.
