
Clinical Pharmacist, Pain/Palliative Care

- Conduct thorough medication reviews for patients with complex pain and palliative care needs, taking into consideration their medical history, current medications, and potential drug interactions.
- Collaborate with physicians, nurses, and other healthcare professionals to develop individualized treatment plans for patients.
- Monitor patient responses to medication therapy and make necessary adjustments to ensure optimal outcomes.
- Educate patients and their families on proper medication use, potential side effects, and medication management strategies.
- Maintain accurate and up-to-date patient medication profiles.
- Ensure compliance with all state and federal regulations related to medication dispensing and administration.
- Participate in multidisciplinary team meetings to discuss patient cases and contribute valuable insights from a pharmaceutical perspective.
- Stay current on the latest developments in pain management and palliative care to provide the best possible care to patients.
- Assist in the development and implementation of hospital-wide protocols and policies related to medication use in pain and palliative care.
- Serve as a resource and mentor for other healthcare professionals on medication-related issues.
- Participate in quality improvement initiatives to enhance patient safety and improve medication management processes.
- Maintain accurate and detailed records of all medication-related activities.
- Adhere to ethical and professional standards in all interactions with patients, colleagues, and other healthcare professionals.
- Continuously evaluate and improve upon personal knowledge and skills through ongoing education and training opportunities.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Valid Pharmacist License In The State Of Massachusetts.
Board Certification In Pharmacotherapy Or Pain Management.
Minimum Of 2 Years Of Experience In A Clinical Pharmacy Role, Preferably In Pain/Palliative Care.
Strong Clinical Knowledge And Understanding Of Pain Management And Palliative Care Treatment Modalities, Including Opioid Therapy And Non-Pharmacological Interventions.
