- Conduct comprehensive medication reviews for patients with cardiovascular conditions to ensure appropriate medication use and identify any potential drug interactions or adverse effects.
- Develop and implement medication therapy management plans in collaboration with the interdisciplinary team to optimize patient outcomes.
- Provide drug information and education to patients, caregivers, and healthcare providers on medication usage, side effects, and potential drug interactions.
- Participate in multidisciplinary rounds and provide recommendations on appropriate medication therapy for patients in the Cardiology department.
- Monitor patient responses to medication therapy and adjust dosages as needed to achieve desired outcomes.
- Collaborate with physicians and other healthcare professionals to develop treatment plans and make recommendations for medication changes.
- Maintain accurate and timely documentation of medication therapy interventions and patient progress.
- Stay up-to-date with the latest developments in Cardiology and pharmaceutical care to ensure the most effective treatment options are being utilized.
- Adhere to all hospital policies and procedures, as well as state and federal regulations, related to medication management.
- Serve as a resource for other healthcare professionals and provide guidance on medication-related issues and concerns.
- Participate in quality improvement initiatives and research projects related to Cardiology and medication management.
- Maintain a high level of professionalism and confidentiality when dealing with sensitive patient information.
- Continuously strive to improve patient outcomes and enhance the overall quality of care provided in the Cardiology department.
- Act as a mentor and preceptor for pharmacy students and residents, providing guidance and support in their clinical rotations.
- Collaborate with pharmacy leadership to develop and implement new programs and services to meet the changing needs of our patient population.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program
Board Certification In Pharmacotherapy Or Cardiology Pharmacy (Bcps Or Bccp)
Current State Pharmacist License In Massachusetts
Minimum Of 2 Years Of Experience In Clinical Pharmacy, Preferably In A Cardiology Setting
Knowledge And Proficiency In Medication Therapy Management, Drug Interactions, And Cardiovascular Pharmacotherapy Guidelines.
