
Case Manager (Per Diem)

- Provide comprehensive case management services to patients and their families, ensuring their needs are met and their care is coordinated.
- Conduct initial assessments and develop individualized care plans for each patient, identifying their goals and needs.
- Collaborate with healthcare professionals and community resources to ensure patients receive appropriate and timely services.
- Monitor and evaluate patient progress and update care plans as needed, advocating for necessary interventions or adjustments.
- Serve as a liaison between patients, families, and healthcare providers, facilitating communication and ensuring all parties are informed and involved in the patient's care.
- Educate patients and their families on healthcare systems and available resources, empowering them to make informed decisions about their care.
- Maintain accurate and thorough documentation of all interactions and services provided to patients.
- Participate in interdisciplinary team meetings and contribute to the development and implementation of hospital-wide initiatives to improve patient care.
- Stay up-to-date on industry developments and changes in healthcare policies and regulations, ensuring compliance with all applicable standards.
- Demonstrate a commitment to providing exceptional care and support to patients and their families, upholding the hospital's mission and values.
Bachelor's Degree In Social Work, Nursing, Or A Related Field.
Minimum Of 3 Years Of Experience In Case Management, Preferably In A Hospital Or Healthcare Setting.
Knowledge And Understanding Of Local, State, And Federal Regulations Related To Healthcare And Social Services.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Patients, Families, And Interdisciplinary Teams.
Strong Organizational Skills And The Ability To Manage A High Caseload And Prioritize Tasks Effectively.
