  1. Provide individualized and comprehensive care to patients in the Partial Hospitalization Program.
  2. Collaborate with a multidisciplinary team to assess and develop treatment plans for patients.
  3. Conduct thorough assessments to determine patients' needs and develop appropriate interventions.
  4. Utilize evidence-based practices and interventions to promote positive outcomes for patients.
  5. Maintain accurate and timely documentation of patient progress and treatment plans.
  6. Advocate for patients and provide support and resources to address their social, emotional, and psychological needs.
  7. Participate in case conferences and team meetings to discuss patient progress and develop treatment goals.
  8. Continuously monitor and evaluate the effectiveness of treatment plans and make necessary adjustments.
  9. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  10. Stay up-to-date with current social work practices and participate in ongoing training and professional development opportunities.
  11. Demonstrate a commitment to cultural competence and inclusivity in working with diverse patient populations.
  12. Collaborate with community resources and agencies to provide additional support and services for patients.
  13. Adhere to Mainehealth's policies and procedures and promote a safe and positive work environment.
  14. Communicate effectively and professionally with patients, families, and colleagues.
  15. Embody the values of empathy, respect, and inclusivity in all interactions with patients and colleagues.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Institution.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In A Behavioral Health Or Mental Health Setting.

  • Strong Understanding Of Mental Health Diagnoses And Treatment Modalities.

  • Ability To Work Collaboratively With A Multidisciplinary Team And Provide Effective Case Management And Support Services.

  • Knowledge Of Relevant Laws, Regulations, And Ethical Standards Related To Social Work Practice.
