Education And Certification: A Minimum Of A High School Diploma Or Equivalent Is Required For This Position. Additionally, Candidates Should Have Completed A Certified Pharmacy Technician Program And Hold A Valid Pharmacy Technician Certification (Cpht) From The Pharmacy Technician Certification Board (Ptcb).
Knowledge Of Medication Reconciliation Processes: Candidates Should Have A Thorough Understanding Of Medication Reconciliation Processes, Including The Ability To Accurately Collect And Document Medication Lists And Identify Discrepancies.
Attention To Detail: As Medication Reconciliation Involves Working With Sensitive Patient Information And Medications, It Is Essential For Candidates To Have A Strong Attention To Detail And The Ability To Accurately Enter Data Into Electronic Health Records (Ehr) Systems.
Communication Skills: This Role Involves Communication With Patients, Healthcare Providers, And Other Members Of The Healthcare Team. Candidates Should Possess Strong Verbal And Written Communication Skills To Effectively Convey Medication Information And Address Any Discrepancies Or Concerns.
