  1. Develop and implement individualized care plans for elderly patients, taking into consideration their specific needs and challenges.
  2. Provide emotional support and guidance to elderly patients and their families, helping them navigate the challenges of aging and promoting a positive outlook.
  3. Collaborate with interdisciplinary healthcare teams to ensure the best possible care for elderly patients, including physicians, nurses, social workers, and other specialists.
  4. Educate patients and their families on healthy aging practices and resources available to them, such as community programs and support groups.
  5. Conduct comprehensive assessments of elderly patients, identifying any physical, cognitive, or emotional limitations and creating interventions to address them.
  6. Monitor and evaluate the progress of elderly patients, making necessary adjustments to care plans and interventions as needed.
  7. Advocate for the needs and rights of elderly patients, ensuring they receive the best possible care and treatment.
  8. Stay up-to-date on current research and best practices in geriatric care, incorporating new knowledge and techniques into patient care.
  9. Document all patient interactions and interventions accurately and thoroughly, maintaining confidentiality and adhering to HIPAA regulations.
  10. Participate in staff meetings and training opportunities to share knowledge and contribute to the overall improvement of elder care services.
  11. Serve as a resource and mentor to other healthcare professionals, providing guidance and support in caring for elderly patients.
  12. Continuously evaluate and improve the Elder Life Specialist program, identifying areas for growth and implementing new initiatives to better serve the community's elderly population.

Job Qualifications
  • Minimum Of Bachelor's Degree In Social Work, Psychology, Or Related Field.

  • Certified As A Geriatric Care Manager Or Equivalent Certification.

  • Minimum Of 3 Years Experience Working With Older Adults And Their Families.

  • Knowledge Of Community Resources And Services For Older Adults.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Team Members.
