If so, we have an exciting opportunity for you to join our team as a Clinical Pharmacist in Primary Care at MaineHealth.As a Clinical Pharmacist, you will play a vital role in providing high-quality, evidence-based pharmaceutical care to patients in a primary care setting. You will work closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and ensure safe and effective use of medications.   1. Provide high-quality, evidence-based pharmaceutical care to patients in a primary care setting.
  2. Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy and ensure safe and effective use of medications.
  3. Utilize strong clinical knowledge and skills to assess medication regimens and make appropriate recommendations.
  4. Conduct medication therapy management and medication reconciliation for patients.
  5. Monitor patient response to medication therapy and adjust as needed.
  6. Educate patients and healthcare team members on safe and effective use of medications.
  7. Maintain accurate and up-to-date medication records and documentation.
  8. Stay current with emerging pharmaceutical trends and new medications.
  9. Assist in the development of clinical protocols and guidelines.
  10. Participate in interdisciplinary team meetings and contribute to patient care plans.
  11. Collaborate with insurance companies and pharmacy benefit managers to ensure proper coverage and reimbursement for patients.
  12. Adhere to all state and federal laws and regulations related to pharmacy practice.
  13. Maintain a patient-centered approach and advocate for patients' medication needs.
  14. Serve as a resource for medication-related questions and concerns from patients and healthcare team members.
  15. Participate in quality improvement initiatives to promote optimal medication use and improve patient outcomes.
  16. Continuously evaluate and improve clinical processes and workflows.
  17. Mentor and train pharmacy students and residents.
  18. Maintain a positive and professional demeanor while representing the organization.
  19. Attend conferences and continuing education opportunities to enhance knowledge and skills.
  20. Promote a culture of safety and quality within the organization.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School

  • Active And Unrestricted License To Practice Pharmacy In The State Of Maine

  • Completion Of A Postgraduate Residency Program In Ambulatory Care Or Primary Care Pharmacy

  • Minimum Of 2-3 Years Of Experience In A Clinical Pharmacy Role, Preferably In A Primary Care Setting

  • Strong Knowledge Of Medication Therapy Management, Drug Interactions, And Evidence-Based Treatment Guidelines For Common Chronic Diseases
