Job Description

We are seeking a highly motivated and dedicated individual to join our team as a Clinical Pharmacist at Mainehealth.   1. Analyze and evaluate patient medication orders to ensure appropriate dosing, interactions, and contraindications.
  2. Collaborate with healthcare professionals to develop and implement medication therapy plans.
  3. Monitor patient responses to medication therapy and make necessary adjustments.
  4. Provide medication education to patients and healthcare staff.
  5. Maintain accurate and complete patient records.
  6. Participate in interdisciplinary rounds and meetings to discuss patient care and medication management.
  7. Stay up-to-date with current pharmacological research and advancements.
  8. Identify and report any medication errors or adverse drug reactions.
  9. Ensure compliance with all state and federal regulations related to medication management.
  10. Assist in the development and implementation of hospital policies and protocols related to medication use.
  11. Serve as a resource for healthcare professionals, patients, and their families regarding medication questions or concerns.
  12. Participate in quality improvement initiatives to promote safe and effective medication use.
  13. Maintain a high level of professionalism and confidentiality in all aspects of the job.
  14. Attend continuing education programs to maintain licensure and stay current with industry developments.
  15. Act as a mentor and preceptor to pharmacy students and residents.
  16. Collaborate with the pharmacy team to optimize workflow and ensure efficient and accurate dispensing of medications.
  17. Adhere to ethical and legal standards in all aspects of job responsibilities.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.

  • Active And Unrestricted Pharmacy License In The State Of Maine.

  • Completion Of A Pharmacy Residency Program Or Equivalent Clinical Experience.

  • Board Certification In Pharmacotherapy Or A Related Specialty (E.g. Ambulatory Care, Critical Care).

  • Excellent Communication Skills And Ability To Collaborate With Healthcare Teams To Optimize Patient Outcomes.
