  1. Coordinate and manage the care of patients, working closely with healthcare providers to ensure the best possible treatment and support.
  2. Conduct assessments of patients' physical, mental, and emotional health to develop individualized care plans.
  3. Collaborate with interdisciplinary teams to ensure all aspects of a patient's healthcare needs are addressed.
  4. Advocate for patients and their families, ensuring their voices are heard and their needs are met.
  5. Monitor patient progress and make necessary adjustments to care plans.
  6. Communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
  7. Utilize knowledge of community resources and support services to connect patients with additional resources as needed.
  8. Maintain accurate and detailed patient records.
  9. Stay up-to-date on industry regulations and best practices related to case management.
  10. Participate in ongoing training and development opportunities to enhance skills and knowledge.
  11. Adhere to all confidentiality and ethical standards.
  12. Provide emotional support and crisis intervention to patients and families as needed.
  13. Coordinate discharge planning and post-hospital care.
  14. Collaborate with insurance companies and other third-party payers to ensure timely and appropriate care.
  15. Participate in quality improvement initiatives to enhance patient outcomes.
  16. Maintain a positive and professional attitude, promoting a culture of compassion and dedication within the healthcare team.
  17. Attend and actively participate in team meetings and case conferences.
  18. Serve as a resource and mentor to other healthcare team members.
  19. Continuously seek opportunities to improve systems and processes related to patient care.
  20. Adhere to all safety and infection control protocols.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Counseling, Psychology, Or Related Field.

  • Minimum Of 2-3 Years Experience In Case Management, Preferably In A Healthcare Setting.

  • Knowledge Of And Experience With Mental Health And Substance Abuse Treatment, Including Knowledge Of Community Resources And Insurance Coverage.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Diverse Team And Effectively Advocate For Patients.

  • Ability To Assess And Address Complex Patient Needs, Develop Treatment Plans, And Monitor Progress Towards Goals.
