Job Description

This position requires a compassionate and empathetic individual who can build strong relationships with clients and collaborate effectively with community partners.To be successful in this role, you must have a Bachelor's degree in social work, psychology, or a related field, as well as at least 2 years of experience in case management.   1. Provide case management services to individuals with disabilities, including assessing their needs, developing individualized service plans, and monitoring progress towards goals.
  2. Collaborate with community partners to ensure clients have access to necessary resources and services.
  3. Conduct regular meetings and check-ins with clients to provide support, guidance, and advocacy.
  4. Maintain accurate and up-to-date client records, including progress notes and service plans.
  5. Utilize a person-centered approach to support clients in achieving their personal goals and promoting their independence.
  6. Stay updated on current community resources and services available to individuals with disabilities.
  7. Serve as a liaison between clients, their families, and community organizations to ensure effective communication and coordination of services.
  8. Advocate for clients' rights and needs, and assist them in navigating systems and processes.
  9. Collaborate with other professionals, such as social workers, therapists, and healthcare providers, to provide comprehensive care for clients.
  10. Attend regular team meetings and training sessions to enhance knowledge and skills related to working with individuals with disabilities.
  11. Develop and maintain strong relationships with clients, building trust and rapport to support their overall well-being.
  12. Adhere to ethical standards and maintain confidentiality in all interactions with clients and their families.
  13. Monitor and report on client progress and outcomes, and make adjustments to service plans as needed.
  14. Participate in program evaluation and quality improvement initiatives.
  15. Act as an advocate for Mainehealth and promote its mission and values in all interactions with clients and community partners.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 2 Years Experience In Case Management Or Community Integration Services.
