Care Management Social Worker - Adult Inpatient

- Provide compassionate and empathetic support to patients and their families during their hospital stay.
- Advocate for the needs and rights of patients, ensuring they receive the best possible care.
- Collaborate with interdisciplinary team members to develop and implement comprehensive care plans.
- Conduct thorough assessments of patients' social, emotional, and psychological needs.
- Connect patients with appropriate community resources and support services.
- Monitor and evaluate patients' progress and adjust care plans as needed.
- Communicate with patients and their families to provide education, support, and counseling.
- Maintain accurate and timely documentation of patient interactions and care plans.

Master's Degree In Social Work From An Accredited Program: A Care Management Social Worker At Mainehealth Would Be Required To Have A Master's Degree In Social Work, Demonstrating A Strong Foundation In Social Work Theory And Practice.
State Licensure: The Ideal Candidate For This Role Would Possess A Current And Valid State License As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw) In The State Of Maine.
Experience In A Healthcare Setting: Candidates Should Have At Least 3-5 Years Of Experience Working As A Social Worker In An Inpatient Healthcare Setting, Preferably With Adult Patients. This Experience Should Include Developing And Implementing Care Plans, Conducting Psychosocial Assessments, And Providing Supportive Counseling To Patients And Families.
Knowledge Of Care Coordination And Discharge Planning: The Care Management Social Worker Will Be Responsible For Coordinating With The Interdisciplinary Team To Develop And Implement Individualized Care Plans For Patients. Therefore, Candidates Should Have A Strong Understanding Of Care Coordination And Discharge Planning Processes.
Strong Communication And Interpersonal Skills: This Role Requires Regular Communication With Patients, Families, And Members Of The Healthcare Team. The Ideal Candidate Should Possess Excellent Communication And Interpersonal Skills, As Well As The Ability To Work Collaboratively With Others To Achieve Common Goals.
