
Hospice Case Manager (RN)
MaineGeneral Health seeks a compassionate and experienced Hospice Case Manager (RN) to provide high quality care to the hospice population. As part of the interdisciplinary team, the Hospice Case Manager will coordinate and manage the care of hospice patients and their families. We are looking for a highly motivated, organized individual with excellent communication and problem-solving skills to join our team. The successful candidate must have a current Registered Nurse licensure in the State of Maine and a minimum of two years of experience in hospice, home health, long-term care, or acute care. Additional qualifications include strong clinical assessment and problem-solving skills, the ability to work independently and collaboratively with the interdisciplinary team, and the ability to manage multiple tasks and prioritize activities. The Hospice Case Manager must also have a commitment to providing compassionate care to the hospice population.
Responsibilities:
- Provide high-quality care to hospice patients and their families.
- Coordinate and manage the care of hospice patients and their families.
- Utilize excellent communication and problem-solving skills to work on an interdisciplinary team.
- Demonstrate strong clinical assessment and problem-solving skills.
- Work independently and collaboratively with the interdisciplinary team.
- Manage multiple tasks and prioritize activities.
- Provide compassionate care to the hospice population.
- Maintain current Registered Nurse licensure in the State of Maine.
