At Logan Health, we are committed to providing compassionate, quality care to those at the end of life. We are seeking an experienced, dedicated Registered Nurse (RN) to join our Hospice team. The ideal candidate will possess the empathy, communication skills, and clinical knowledge necessary to provide exceptional care for our patients and their families during this difficult time.The RN - Hospice will partner with the interdisciplinary team to provide comprehensive patient-centered care. The RN - Hospice will assess, plan, coordinate, provide, and evaluate care for patients with terminal illness and their families. This role requires a strong understanding of the physical, psychological, social, spiritual, and cultural needs of the patient and their family.To be successful in this role, you must have a current Registered Nurse license in the state of practice, a minimum of two years of nursing experience, and knowledge of and experience in hospice care. You must also possess strong interpersonal skills and the ability to provide spiritual support. Responsibilites of an RN - Hospice:
• Assess, plan, coordinate, provide, and evaluate care for patients with terminal illness and their families. • Develop and implement care plans to meet the physical, psychological, social, spiritual, and cultural needs of the patient and their family. • Monitor patient health status and adjust care plans as needed. • Administer medications and treatments according to physician orders and standards of care. • Provide education and support to patients and their families. • Provide spiritual and emotional support to patients and families. • Maintain accurate and detailed records of patient care and condition. • Collaborate with the interdisciplinary team to ensure optimal patient outcomes.
Excellent Communication And Interpersonal Skills
Licensed Registered Nurse
At Least Two Years Of Experience In Hospice Or Home Health Nursing
Ability To Provide Compassionate Care To Terminally Ill Patients
