
Physical Therapist - Home Health

Logan Health is looking for a Physical Therapist who is committed to the highest quality of care to join our Home Health team. In this vital role, you will provide physical therapy services to patients in the home setting. To be successful in this role, you must have a Bachelor’s or Master’s Degree in Physical Therapy from an accredited school, a valid state license or certification, and a minimum of one year of experience in physical therapy. You should also have strong clinical assessment skills and knowledge of home health regulations. Responsibilities:
- Provide physical therapy services to patients in the home setting.
- Demonstrate strong clinical assessment skills and knowledge of home health regulations.
- Develop individualized treatment plans for patients based on their specific needs.
- Monitor and evaluate patients' progress to ensure desired outcomes.
- Provide education and support to patients and their families about physical therapy services.
- Collaborate with other members of the home health team to provide comprehensive and coordinated care.
- Maintain accurate and complete records of patients' treatment and progress.
- Adhere to all state and federal regulations governing physical therapy.
Bachelor's Degree In Physical Therapy From An Accredited College Or University
Current License As A Physical Therapist In The State Of Montana
At Least Two Years Of Experience In Home Health Physical Therapy Or Equivalent
Evidence Of Continuing Education In Physical Therapy
Ability To Use Electronic Medical Records Systems
Knowledge Of Pain Management And Physical Therapy Modalities
