CNA - Hospice/Home Health

Responsibilities:

1. Provide high quality personal care to patients in a home setting.
2. Assist patients with daily activities such as bathing, dressing, and grooming.
3. Monitor and record patients’ vital signs and other medical information.
4. Assist patients with mobility, positioning, and transferring.
5. Administer medications to patients as needed.
6. Maintain clean and safe environment for patients.
7. Provide emotional support and promote positive patient relationships.
8. Create and implement individual care plans according to patient needs.
9. Communicate effectively with patients, their families, and other healthcare professionals.
10. Follow all health and safety regulations and policies.
