
CNA - Hospice/Home Health

Logan Health is seeking an experienced, compassionate, and reliable Certified Nursing Assistant (CNA) to join our Hospice and Home Health team. As a CNA, you will have an opportunity to make a difference in the lives of our patients and their families. The ideal candidate will have a minimum of one year of experience in the healthcare field, with a current CNA certification. You should be comfortable providing personal care in a home setting, possess excellent communication skills, and demonstrate the utmost professionalism. Above all, you must be dedicated to providing the best possible care and support to our patients.
Responsibilities:
- Provide high quality personal care to patients in a home setting.
- Assist patients with daily activities such as bathing, dressing, and grooming.
- Monitor and record patients’ vital signs and other medical information.
- Assist patients with mobility, positioning, and transferring.
- Administer medications to patients as needed.
- Maintain clean and safe environment for patients.
- Provide emotional support and promote positive patient relationships.
- Create and implement individual care plans according to patient needs.
- Communicate effectively with patients, their families, and other healthcare professionals.
- Follow all health and safety regulations and policies.
